A growing trend that provides virtual care to patients at their homes is famous among the health systems and hospitals. Trinity Health just joined this trend. 93 hospitals and 120 counting care programs including the hospice services, senior living facilities,PACE and home care are included in the Livonia, Michigan-headquartered Trinity. Numerous and frequent medical complications have to be monitored in case of the patients in home care. To keep patients well and out of the hospital, reduce readmissions and ER visits is the primary aim of the initiative, Trinity Health At Home. Including wireless 4G-enabled tablet designed with text and voice directions that walk users through the steps for observing and reporting on their current wellness status and for taking part in virtual visits and patient education programs, the designed product can be used out of the box.
John Capasso, executive vice president of Trinity Health Continuing Care said in a statement that primary objective is to deliver all Trinity Health At Home patients with care that is proactive, compassionate and transforming while keeping them in their own homes. Including blood pressure, weight and other relevant measurements while connected to the additional healthcare devices.
